FREE HEALTH SCREENINGS FOR GRADES K, 1, 3, & 6

Per Florida Statutes, Section 64F-6.003 Screening, Charlotte Academy is required by the State of Florida Department of Health to offer free health screenings to students in grades K, 1, 3, and 6. Screenings are conducted for vision, hearing, height and weight for all students, as well as for scoliosis in grade 6. A licensed physician will conduct the screenings, assisted by school staff members who have received the appropriate training from the Charlotte County Department of Health. Screenings will be conducted on Friday, February 24, 2012. If you would like your child to be screened, please sign and return this permission slip by Monday, February 20

AMERICAN SIGN LANGUAGE AFTER-SCHOOL CLASS
Delta students in grades 4-8 are invited to sign up for American Sign Language classes taught by CA parent and Nationally Certified Sign Language Interpreter Mrs. Sharon King, MA CI/CT. Class begins Wednesday, January 18, 2012. Click here for the informational flyer!

Please forward completed forms to the Front Office. Approved forms are valid for the entire school year, approval is not transferable to other drivers or vehicles.
